The information included in this guide only applies to the employees represented by the unions and associations that have reached an agreement.
Employees represented by the unions that have not reached an agreement will be notified of their enrollment at a later time.
This is a changes only enrollment. If you would like to continue with your current benefits selection for the 2016 plan year, you DO NOT need to enroll, your current benefits will continue and premiums and plan designs changes will automatically be adjusted effective January 1, 2016. Your period of coverage dates are: January 1, 2016, through December 31, 2016.
