If you are currently enrolled in any of the flexible benefits, you do not need to submit an enrollment form. Your benefits will continue. NOTE: There are no plan design or premium changes to these plans.

 

To enroll in any of the flexible benefits, effective January 1, 2017, you must request a flexible benefits enrollment form. Mail your completed form, with your first month’s premium, payable to FBMC Benefits Management, Inc. to:
